Frederick Health Medical Group (FHMG), a highly respected 160+ provider, multi-specialty medical group, is seeking an experienced Nurse Practitioner or Physician Assistant - to serve as the primary clinical navigator and care coordinator for patients undergoing genitourinary (GU), gastrointestinal (GI), and thoracic cancer surgery.

This is a high-impact, relationship-driven role designed for an APP who thrives on continuity, coordination, and deep patient engagement. In this position, you are the single, trusted point of contact for each patient, guiding them seamlessly through pre-operative, peri-operative, and post-operative care within the Multidisciplinary Clinic (MDC) model.

  • Coordinate and oversee care across surgeons, oncology specialists, diagnostics, and follow-up services to ensure seamless, timely care
  • Perform focused histories and physical exams; assess, diagnose, and manage patient needs using evidence-based guidelines
  • Educate and support patients and families, addressing medical, psychosocial, and behavioral health needs
  • Monitor changes in patient status and proactively collaborate with the care team to adjust care plans
  • Document care accurately and timely in the EMR while ensuring regulatory, quality, and coding compliance
  • Participate in multidisciplinary collaboration and quality improvement initiatives
