The Resident Care Coordinator supports the delivery of high-quality resident care by coordinating assessments, care plans, staffing support, and communication with residents, families, and care providers. This role partners closely with community leadership and the interdisciplinary team to promote resident dignity, safety, independence, and quality of life while ensuring compliance with all applicable regulations.
Key ResponsibilitiesResident Care & Coordination
- Complete and support pre-move-in, move-in, and ongoing resident assessments
- Assist in developing, updating, and monitoring individualized care plans
- Serve as a primary point of contact for residents and families regarding care needs
- Monitor changes in resident condition and communicate concerns appropriately
- Coordinate care with physicians, pharmacies, hospitals, and external providers
Team Support & Oversight
- Support care team scheduling and staffing coverage as needed
- Provide guidance and support to care team members
- Assist with onboarding and ongoing training, including dementia-specific education
- Promote teamwork and alignment with Solera’s Core Values
Medication & Documentation
- Support medication management processes, including coordination with pharmacy services
- Ensure care documentation is accurate, timely, and compliant
- Assist with medication storage, documentation, and compliance oversight
Compliance, Safety & Quality
- Support compliance with federal, state, and company care regulations
- Participate in surveys, inspections, and quality assurance initiatives
- Maintain a safe and secure environment for residents, team members, and guests
- Assist with emergency preparedness and safety procedures
Community & Operational Support
- Participate in daily stand-up meetings and interdisciplinary collaboration
- Support tours, referrals, and census growth initiatives as needed
- Assist with supply tracking and care-related inventory
- Promote a positive, professional community culture
Education & Experience
- Preferred:
Two (2) years of college coursework in healthcare, human services, or a related field - Preferred:
Four (4) years of experience in senior living, residential care, or dementia-related services
Skills & Abilities
- Strong organizational, communication, and problem-solving skills
- Ability to manage multiple priorities in a resident-focused environment
- Proficiency with basic office systems and electronic care documentation
