Summary
Under the direction of a Registered Nurse (RN), the Licensed Practical Nurse delivers efficient and effective nursing care while achieving positive clinical outcomesandpatient/familysatisfaction.
He/sheoperateswithinthescopeofpracticedefinedby the State Nurse Practice Act.
The LPN contributes to nursing assessments and care planning, provides direct patient care, and supervises patient care provided byunlicensed staff.
- Collects,reportsanddocumentsobjectiveandsubjectivedata;
- Care Planning:
- Assistsindevelopinginterventionstoachievegoals;
- Implementstheplanofcare;
- Evaluateseffectivenessofinterventionstoachievepatientgoalsandminimizere-hospitalizations;
- Participatesinreviewandrevisionofplanofcare;
- Administersmedicationsandperformstreatmentsperphysicianorders;
- Documentsaccuratelyandthoroughly;
- Consultsandseeksguidancefromthe
RNasnecessary; - Atthedirectionofthe
RN,participates in
Utilization Manage mentor Interdisciplinary Care Management Meeting to facilitate appropriate discharge planning: - Communicatesteam'srecommendationsfordischargetotheattendingphysician, patient andfamily;
- Monitorspatientcareprovidedbyunlicensedstaff:
- Communicatespatientinformationwithunlicensedstaffduringandbetweenshifts;
- Ensuresthatassignedtasksareperformedinaccordancewithpoliciesandprocedures;
- Uses Point Click Care (PCC) accordingtothe
Business
Processes; - Performsdrugcountwhenbeginningandendingshiftandwhenevernecessary;
- Assistsphysicianinroundsandexaminationofpatients,asneeded;
- Collaboratesandcoordinateswithotherdepartmentstoprovidetimelyeffectivecare consistent with individual's needs, choices and preferences;
- Contributestoanenvironmentthatisrespectful,team-oriented,andresponsivetothe concerns of staff, patients and families;
- Enhancesnursingpracticebyattendingallmandatedin-serviceprograms;
- Assistswiththeorientationofnewlyhirednursingstaff;
- Staysandworksbeyondscheduledshiftifneededtomeetstatestaffingrequirementsand/or needs of patients;
- Performsotherdutiesasrequested.
Complieswithapplicablelegalrequirements,standards,policiesandproceduresincludingbut not limited to those within the Compliance and Ethics Program, Standard/Code of Conduct, Federal False Claims Act and HIPAA.
Promptlyreportsconcernsandsuspectedincidencesofnon-compliancetosupervisor, Compliance Liaison or to the Compliance Officer via the Integrity Hotline.
Cooperateswithmonitoringandauditfunctionsandinvestigations.
Participates,as requested,inqualityassuranceandprocessimprovementactivities.
Specific Educational/Vocational RequirementsMustbecurrentlylicensedbythe State Boardof Nursing .
This position requires that the employee is able toread, write, speak and understand the spoken
Englishlanguagetoensurethesafetyandwellbeingofourpatientsandvisitorsatthe work site when responding to their medical and physical needs.
Must provide verification of TST (tuberculin skin test) as required bystate law and in accordance with
Company policy.
TSTswillbeadministeredattheworksiteifrequired.
Maintains current
BLS/CPR certification(AllSNFsand Behavioral Centers ;
ALFs/ILFsas required by State).
- Comprehensiveknowledgeofnursingprinciplesrequired,includingtheabilitytorecognize and identify symptoms and manage emergency situations.
- Knowledgeofmedications,theirproperdosage,andexpectedresults.
