Position: Hospice Chaplain- Full Time

Overview

Position:
Hospice Chaplain/Spiritual Care Provider responsible for providing spiritual care services to patients and families/caregivers of Mission Hospice and Home Care, directly or through coordination with hospice team members. This role also supports staff and volunteers as needed and may serve on the interdisciplinary team.

Responsibilities
  • Provide short-term pastoral counseling to patients, families and staff, using age appropriate communication.
  • Complete assessment of spiritual needs for patients and families/caregivers and render appropriate services in a timely manner.
  • Maintain electronic medical records of spiritual care services provided to patients and families/caregivers in a timely manner.
  • Initiate and update spiritual plans of care (POC) by charting clinical problems, goals and interventions on the comprehensive assessment form and POC.
  • Attend and contribute to patient care conferences and interdisciplinary group meetings.
  • Provide clinical consultation and education to the interdisciplinary group on spiritual care.
  • Extend spiritual support to all staff and volunteers, as needed; serve on the IDT
  • Serve as liaison to community-based chaplains, clergy and spiritual counselors.
  • Officiate funerals or memorial services for families, as requested.
  • Conduct periodic memorial services or other rituals to meet the needs of staff, volunteers and community clergy/spiritual counselors working with the hospice team.
  • Perform other duties as assigned.
Education and Experience
  • Minimum of 2 units Clinical Pastoral Education (CPE) required.
  • Minimum of 2 years hospice experience (.5 or 1 FTE) required.
Skills and Abilities
  • Knowledge of different philosophical systems and religions. This includes their basic principles, values, ethics, ways of thinking, customs, practices, and their impact on human culture.
  • Apply culturally appropriate, evidence-informed strategies for addressing the breadth and depth of multifaceted grief including complicated and anticipatory grief to extend to bereavement resources.
  • Ability to work independently and take initiative.
  • Good judgment and problem-solving skills.
  • Demonstrate competency in the cultural and age specific needs of the clients.
  • Demonstrate and model the ability to attend to the physical, emotional, social and spiritual well-being of the transdisciplinary team.
  • Demonstrate a working knowledge of the difference in the provision of advanced practice chaplaincy care as care recipients and families negotiate through the trajectory of a life-threatening illness in various settings.
  • Knowledge of MS Office programs.
  • Current drivers’ license required.
  • Proof of automobile insurance required.
Physical Demands/Work Environment
  • Job requires some physical activities including but not limited to; sitting or standing for extended periods of time, typing, reaching, standing, walking, kneeling, lifting, fingering, grasping, feeling, talking and hearing.
  • Job requires use of general office equipment including phones, copiers and computer equipment.
  • Job requires light physical work including the ability to exert up to 10 lbs. of force occasionally, 5 lbs. regularly.
  • Job requires visual acuity including the ability to see at or within arm’s reach as well as to operate a motor vehicle.
  • Job is subject to inside environmental conditions; low noise level.
