We are seeking dedicated and compassionate Licensed Practical Nurses (LPNs) to deliver high-quality nursing care and provide direct supervision to nursing assistants. In this role, you will support our residents  overall health and well-being while ensuring compliance with facility standards and state/federal regulations.

• Provide direct nursing care to residents in accordance with care plans and physician orders
• Supervise Certified Nursing Assistants (CNAs) during daily care activities
• Administer medications and treatments as prescribed
• Monitor residents  conditions and report changes to the RN or physician
• Participate in care planning, charting, and documentation
• Assist in admissions, discharges, and resident assessments
• Maintain a clean, safe, and supportive environment
• Communicate effectively with residents, families, and interdisciplinary teams
• Assist with wound care, Braden assessments, and skin condition monitoring
• Maintain accurate medication and narcotic records
• Ensure compliance with infection control and safety protocols
• Support emergency preparedness and evacuation procedures

• Valid New York State LPN license (required)
• Graduate of an approved LPN program
• Prior experience in long-term care or rehabilitation setting preferred
• Strong clinical skills and a solid understanding of nursing practices
• Excellent communication, organization, and supervisory skills
