Licensed Practical Nurse (LPN) – 11 PM to 7 AM Shift

• Per diem rate: $42.00 plus differential
• Sign‑on bonus: $2,500.00 (prorated for part‑time)

• Current, unencumbered license to practice nursing in the state where the center is located
• Proficiency in basic computer skills, including electronic health records; PCC experience is a plus

• Implement systems that address each resident’s needs, with broad knowledge to manage medically complex and mentally compromised residents
• Assess residents’ health care needs, develop a nursing diagnosis, transition this into an interdisciplinary plan of care, and communicate the plan to all care providers to ensure continuity of care
• Establish and maintain functional communication with staff, other disciplines, residents, families, medical staff and community
• Treat residents, staff, and the facility community with respect and dignity, and handle resident and facility property and information with honesty, confidentiality, and respect
