Onboarding Process: Please be advised that candidates must successfully complete a background check and pre-employment health screening which includes a drug screen.
Responsibilities- Complex psycho-social transition planning, assessment, reassessment, and intervention.
- Assistance with adoptions, abuse and neglect cases, including assessment, intervention, and referral as appropriate to local, state, and/or federal agencies.
- Care coordination and oversight of the transition plan; lead and/or facilitate multi-disciplinary patient care conferences including Complex Case Review.
- Communicate with patients and families about the plan of care; collaborate with physicians, office staff, and ancillary departments.
- Ensure patient education supports post-acute needs and complete timely, concise documentation in the Case Management system.
- Maintain accurate patient demographic and insurance information; precept new staff members and act as a resource to all staff.
- Facilitate TEMPO as needed; participate in department quality improvement initiatives; perform other duties as assigned.
Education: Required:
Master’s of Social Work
Experience: Preferred: 2 years of acute hospital experience
Certifications/Licensure: Must be currently licensed or license eligible to practice as a LSW or LCSW in adherence with state regulatory requirements
