Position Responsibilities:
Essential

- Facilitates the admission process, informs new residents and families of the rules and expectations of the program, and actively engages the family with regular communication to establish a mutually trusting and cooperative relationship.
- Presents a caseload and participates in weekly staffing team meetings.
- Participates in weekly supervision with the Program Director.
- Documents all services in compliance with State Standards, adhering to established time frames, styles and content.
- Generates and files Care Plans/Reformulations according to State Standards and within the designated time frames.
- Facilitates and co-facilitates groups (Process Group), family meetings and individual sessions and documents appropriately in the Weekly Progress Summary Notes.
- Administers and files standardized assessments as assigned
- Schedules and documents all visits to maintain contact with the referring agency and family.
- Arranges, supervises and documents telephone calls for assigned residents with referring agencies, families and other authorized parties.
- Screens in-coming and out-going mail according to policy and procedures.
- Conducts transition planning and assessments of needs beyond aftercare service period.
- Schedules in conjunction with the Program Director, discharge dates for all assigned residents and notifies family, referring agency, and other necessary parties of pending discharge.
- Coordinate and facilitate graduation process for residents who meet achievement criteria to graduate from the program.
- Writes a formal discharge summary on all assigned to his/her caseload within 10 business days following discharge.
- Assumes portions of co-workers’ caseload as assigned, in his/her absence, to encompass all of the above defined duties.
- Participates in on-call rotation and provides front line coverage and crisis intervention, as needed.
- Read the communication book, emails, and incident reports daily.
- Administers and manages medications for caseload.
- Completes bi-weekly audits of the MARs, facilitates correction of identified errors, and communicates this information to the Compliance Director.
- Participates in seminars and other sex offender specific training opportunities as assigned to remain current in latest research development in behavioral services.
- Other duties as assigned.
- Bachelor’s degree in human services, counseling, or related field with a minimum of three years of experience working with emotionally disturbed children.
- Positive and effective communication skills.
- Excellent organizational skills.
- Excellent writing and documentation skills.
- Computer skills to include proficiency in Microsoft Office software.
- Leadership and role modeling abilities.
- Responsive to cultural differences.
- Extensive training and experience with adolescent sexual offenders.
