About the Role

As a Registered Nurse at Stonegate Nursing & Rehabilitation, you will play a critical role in delivering high‑quality, compassionate care to patients in a rehabilitation and long‑term care setting. Your primary focus will be on assessing patient conditions, administering treatments, and coordinating with interdisciplinary teams to develop and implement individualized care plans. You will be responsible for educating patients and their families about health management and recovery processes, ensuring they are well‑informed and supported throughout their care journey.

This role demands vigilance in monitoring patient progress, managing complex medical needs including telemetry and wound care, and advocating for patient rights and well‑being. Ultimately, your contributions will directly impact patient outcomes and enhance the overall quality of care provided by our facility.

Minimum Qualifications
  • Current and valid Registered Nurse (RN) license in the state of Texas.
  • Completion of an accredited nursing program (Associate’s or Bachelor’s degree in Nursing).
  • Basic Life Support (BLS) certification.
  • Minimum of 2 years of clinical nursing experience, preferably in medical/surgical or rehabilitation settings.
  • Demonstrated proficiency in direct patient care and nursing documentation.
Preferred Qualifications
  • Bachelor of Science in Nursing (BSN) degree.
  • Experience working with nurse practitioners and interdisciplinary healthcare teams.
  • Familiarity with electronic health record (EHR) systems used in rehabilitation facilities.
  • Strong background in patient education and advocacy within long‑term care environments.
Responsibilities
  • Provide direct patient care including assessment, monitoring, and documentation of patient health status.
  • Administer medications and treatments as prescribed, with particular attention to wound care and telemetry monitoring.
  • Educate patients and their families on disease management, post‑discharge care, and health maintenance.
  • Collaborate with physicians, nurse practitioners, and other healthcare professionals to develop and update individualized care plans.
  • Advocate for patient needs and preferences, ensuring ethical and patient‑centered care practices are upheld.
  • Maintain compliance with healthcare regulations and facility policies to ensure patient safety and quality standards.
  • Participate in ongoing professional development and training to stay current with best practices in medical and surgical nursing.
