Licensed Vocational Nurse (LVN) – Full‑Time/Per Visit – Home Health

Frontpoint Health is seeking a full‑time LVN to provide skilled nursing care to patients in their homes in the Fort Worth, TX area. The position requires a minimum of 36 productivity points each week and includes participation in a compensated on‑call program.

Description

The LVN will see all visits assigned within the scope of their license, administer skilled nursing care, coordinate with the interdisciplinary team, and maintain accurate clinical records. The role supports patient safety and quality improvement through regular reporting and participation in educational activities.

Job Responsibilities
  • Be available Monday–Friday to take patients.
  • Perform skilled nursing services under the supervision of a staff LVN.
  • Observe, record, and report to the supervisor on the patient’s physical and mental condition.
  • Administer prescribed medications and treatments as permitted by state regulations.
  • Assist the physician and/or staff LVN in performing specialized procedures.
  • Assist the patient with activities of daily living and encourage appropriate self‑care.
  • Prepare clinical and/or progress notes and incorporate them into the clinical record at least weekly.
  • Report to the patient’s physician and other appropriate personnel regarding pertinent changes in the patient’s condition.
  • Perform complex wound care if in‑service is documented for a specific procedure.
  • Perform routine venipuncture to obtain labs only if documentation of competency, by a staff LVN, is in the personnel record.
  • Receive and follow physician orders within the scope of practice for an LVN and within home‑health practice standards, but may not alter the plan of care.
  • Participate in in‑service program, attending at least two educational meetings per year.
  • Participate in nursing report.
  • Participate in staff conference regarding patient care.
  • Participate in QAPI plan and process.
Job Requirements
  • Active state license as a Licensed Vocational Nurse or a multi‑state license issued by the Nurse Licensure Compact.
  • 2–3 years of experience as an LVN.
  • Proficiency in nursing skills as generally accepted in the field; good interpersonal skills.
  • Proof of current CPR, TB, auto insurance, and COVID‑19 vaccine.
  • Reliable transportation.
  • Experience with Kinnser software.
