About

the role

A professional who provides nursing services within the scope of nursing practice standards in collaboration with primary care physicians, teaches and educates clients and their families. Professional skilled nursing is performed under the supervision of the Administrator/Supervising Nurse.

Note:

This position pertains to in-home client services and telehealth in DFW, Texas.

What you'll do

Furnishes those services requiring substantial and specialized nursing skill.

Initiates appropriate preventative and rehab nursing procedures.

Prepares & submits timely clinical and progress notes.

Oversees coordination of client services.

Informs MD and other staff of changes in patient situation or needs.

Completes medical history/home safety check/environmental assessments.

Performs socio-psychological evaluation.

Performs assessment visit and documents timely (e.g., OASIS/Skilled Nursing Note).

Performs physical examination and review of all body systems and documents accordingly.

Develops individualized Plan of Care (POC) to be submitted to the physician for approval and implementation, revising as needed based on client needs.

Determines medical necessity for other services.

Regularly re-evaluates the client’s nursing needs.

Evaluates the client’s ADL and iADL abilities and need for home health aide services.

Develops and implements the HHA plan of care when HHA services are ordered, revising and signing it at each certification period.

Supervises HHA in accordance with state/federal requirements and documents supervision.

Orders “other” professional services as appropriate.

Reviews billing processes with the client and/or family, advising when co-pay or Medicare may not cover services.

Communicates ongoing progression/changes in POC effectively with the client and family.

Communicates with Supervisor, Supervising nurse, and other disciplines as needed.

Contacts the client’s MD (verbally and/or in writing) for effective treatment and therapy modalities.

Participates in case conferencing to establish best practices.

Submits requests for re-authorization of “more visits needed” to payers timely.

Coordinates community services to ensure safe home care needs.

Participates in the QAPI Committee process.

Performs timely CRR per agency policy in collaboration with the Nursing Supervisor.

Participates in staff meetings and in-service trainings.

Acts as a preceptor for new employees in the same discipline.

Contributes to the planning, operation, and evaluation of nursing services within the organization.

Maintains professional licensure per state requirements.

Notifies the agency promptly of emergencies, sickness, or other events affecting client care.

Submits written time requests at least two weeks in advance of planned time off.

Performs other duties as assigned.

Qualifications

Active license to practice professional nursing in Texas.
Preferably completed a Bachelor of Arts degree program.
Minimum of two (2) years of experience as a professional nurse, with at least one (1) year in community/home healthcare preferred.
Ability to pass a criminal background check and any other required checks.
