Position highlights- Work location:
1301 Pennsylvania Ave. Fort. Worth, TX 76104 - Hours:
Day Shift, Monday – Friday
This is a very collaborative practice that includes a mix of inpatient management (perioperative, post‑transplant, rejection management and re‑admissions) as well as the outpatient clinic with both pre‑transplant (patient education) and post‑transplant (medication / disease state management) all in collaboration with a hands‑on multidisciplinary team.
Here’s What You Need- Bachelor’s Degree Pharmacy required
- Pharm
D. degree from a U.S.
-accredited College of Pharmacy required - 1 year PGY1 residency experience upon hire required
- 3 years focused hospital pharmacist experience required
- 2 years residency experience in Renal Transplant (PGY2) required upon hire
- 5 years of hospital pharmacist experience required
- Highly preferred PGY1 residency completion (or equivalence)
- Demonstrated leadership experience, training and development and contributions to the profession required.
- Guide the collective effort towards constant improvement and goal achievement.
- Work independently as well as participate and lead group efforts to improve pharmaceutical care.
- Publishing (posters at regional professional meetings to publishing in national peer‑reviewed journals)
- Participation (minimum) / leadership (at least periodically) in relevant professional organizations.
- Active leadership in preceptorship for resident and student programs
- Maintaining relevant board certification in applicable specialty area
- Other duties as assigned.
