Job SummaryThe Lead Patient Navigator
- Grants trains and helps direct the development and education of a team of navigators, providing guidance and support to individuals at risk of SUDs, HIV/STIs, STIs infection, and coordinating prevention and education efforts within the community. This position involves planning and implementing prevention strategies, conducting outreach and education, and ensuring individuals have access to necessary resources and services.
- Assists with overseeing the work of other team members by training, allocating and monitoring work. Performs work of a similar nature.
- Leads communication of culturally sensitive and evidence-based HIV education and prevention to individuals and groups.
- Collaborates with stakeholders to create and adapt prevention programs to target high-risk populations.
- Assists clients in accessing HIV and substance use prevention services including PEP, PrEP, and other relevant services.
- Develops and maintains professional partnerships with community organizations to increase access to prevention services.
- Leads outreach activities to identify at-risk individuals and promote HIV testing and prevention measures to increase recruitment and retention in the program.
- Connects individuals to healthcare providers, social services, and support networks.
- Assists each patient in developing a network of contacts with family, friends and providers that will commit to actively be involved in the patient s life.
- Connects patients with service providers.
- Assists patients in utilizing service providers by transporting patients to appointments, role-playing, problem solving and being a source of encouragement and inspiration.
- Monitors the quality of services provided and identifies areas for improvement.
- Conducts regular evaluations and assessments to measure program success.

Required Qualifications:
- Bachelor’s degree in Psychology, Healthcare Administration, Sociology or related field of study OR, in lieu of Bachelor’s Degree, HS Diploma and 5+ years of assisting clients in navigating healthcare systems to ensure timely access to prevention and care services.
- 3 plus years of experience in a customer service environment.
- Must complete all training required of Agency new hires, including any training required by TDSHS HIV Care Services Branch Standards of Care and SAMHSA within established time frames.
- Successful completion of the HIV Navigation in Texas Learning Series is required within the first year of employment. This includes pre-requisites, learning modules and the face-to-face Learning Session.
- Successful completion of HIV Prevention in Texas is required within 3 months of employment.
- STD 101:
What You Need to Know (online) required of all contractors within three months of employment. - Valid driver s license and access to transportation.
Preferred Qualifications:
- Bachelor s Degree in Psychology, Healthcare Administration, Sociology or related field of study.
- 1 plus year of experience with community public service experience.
- Bilingual skills preferred.
