Working closely with community agencies, social workers and the Market-Based Population Health teams, at Privia Health, the Community Health Worker will provide assessment of patient needs, make community referrals, and follow up with patients who are identified as benefiting from additional support and intervention from their health care team.

Minimum Qualifications:

  • Education:

    High School Diploma
  • Licensure:
    Certified Community Health Worker (within 1 year of hire date) Company will provide training options
  • Experience:

    Minimum of 1 year of experience in social service or healthcare setting
  • Experience working with at-risk patients is highly desirable.
  • Skills:

    Strong communication, and interpersonal skills.
  • Ability to work in a fast-paced environment and manage multiple priorities.
  • Proficiency in electronic health record (EHR) systems.
  • Competencies:

    Empathy, critical thinking, and the ability to work collaboratively with diverse populations and interdisciplinary teams.

Interpersonal Skills & Attributes:

  • Strong interpersonal skills.
  • Excellent communication, interpersonal, and organizational skills.
  • Ability to work collaboratively in a multidisciplinary team environment.
  • Proficiency in electronic health records (EHR) and care management software.
  • Knowledge of community resources and social service systems.
  • Demonstrate sound judgment and strategic mindset through critical decision-making
  • Maintain a positive, caring attitude toward patients
  • Ability to function efficiently and meet defined goals
  • Must be detail-oriented
  • Possess a focused work ethic and a high level of professionalism
  • Must be able to travel 1-2 times a month to Stephenville, Midlothian or Ennis
  • Self-motivated:
    Identifies opportunities and finds solutions to problems
  • Able to multitask effectively
  • Proficient with Microsoft Office applications including Word, Excel, and PowerPoint as well as Google Docs
