Role SummaryThe Patient Navigator contributes to AHF by providing short-term assistance to clients to improve engagement in the health care system and the HIV Continuum of Care. This role involves coordinating referrals, guiding clients to appropriate services, and collaborating with interdisciplinary staff to address client needs and barriers to care.
Responsibilities- Meet clients where they are and direct them to appropriate services in person, in writing, by telephone, or through other methods of communication.
- Assist with completing initial Ryan White enrollment, six-month attestation, and annual recertification.
- Support enrollment in private insurance via the Affordable Care Act (ACA) for eligible clients, ensuring Ryan White remains the last payer source.
- Initiate referrals to agency and community service providers as needed and conduct follow-up within ten business days to check referral status.
- Collaborate with Medical and Non-Medical Care Managers for clients requiring long-term assistance to address barriers to care and ensure continuity of care.
- Schedule medical appointments and coordinate medical transportation when related to client care.
- Contribute to department-wide projects that support AHF’s mission, focusing on getting clients into care and keeping them engaged.
- Attend staff and interdisciplinary meetings related to client care as part of a close-knit team.
- High School Diploma or GED required. Knowledge of HIV treatment and care guidelines.
- Bilingual:
English/Spanish
