Base pay range: $40.00/hr - $42.00/hr (continuum group pays based on skills and experience).

We are an in‑house therapy team providing care at skilled nursing facilities across Texas, New Mexico, Indiana, Arizona, and Missouri. • Utilize evidence‑based practice to support clinical interventions.
• Provide rehabilitative, skilled and medically necessary treatment interventions to patients with physical functioning disorders per regulatory and clinical practice requirements under the supervision and direction of a Physical Therapist.
• Provide skilled therapy services to patients who need rehabilitation services.
• Assess and document patient progress and communicate with supervising Physical Therapist to adjust treatments accordingly.
• Provide consultation and counseling to patients, families, caregivers and other service providers related to physical disorders.
• Provide input to supervisor for discharge summary of services provided per regulatory and clinical requirements.
