We are seeking PTs who are flexible to go anywhere to our affiliates in 17 states:
Alaska, Arizona, Oregon, Washington, California, Idaho, Nevada, Utah, Colorado, Nebraska, Iowa, Wisconsin, Kansas, Alabama, Texas, South Carolina, and Tennessee.

• Travel only within Ensign affiliated Skilled Nursing and Post-Acute care facilities.

• Effectively screen/evaluate patients within short‑term and long‑term rehab and opportunities for outpatient.
• Utilize standardized assessments and evidence‑based practice to support clinical interventions.
• Provide rehabilitative, skilled, and medically necessary treatment interventions to patients within short‑ and long‑term settings.
• Administer a wide variety of interventions as part of the plan of care based upon individual patient needs.
• Assess patients’ progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
• Provide consultation and counseling to patients, families, caregivers, and other service providers related to the physical disorders.
• Effectively communicate with the One Clinical Team regarding patient progress and barriers.
• Provide comprehensive discharge summary of services provided per regulatory and clinical requirements.

Licensed Physical Therapist.
