Title: Outreach Executive I

Role DescriptionThe Outreach Executive (OE) is responsible for developing relationships with members of the community to engage Medicare seniors with Oak Street Health. OE’s goal is to bring these seniors into Oak Street Health as patients. OEs work closely with Outreach Managers, Outreach Directors, and Community Relationship Managers to drive new patient growth in their assigned territory.
Core Responsibilities- Sales
- Generate leads by engaging Medicare seniors through events and local community outreach.
- Maintain knowledge of internal sales systems and marketing programs by attending scheduled meetings.
- Prospect and cold‑call prospects and new leads assigned to you. Additional leads may come from digital channels or other OEs.
- Nurture prospective patients via email, phone, and networking, maintaining an ambitious mindset.
- Collaborate with Community Relationship Managers, other OEs, and Outreach managers/directors to drive lead generation with local community members or insurance agents.
- Relationship Management
- Maintain consistent touchpoints with prospects to continuously engage them and drive new patient growth.
- Capture and record customer information and sales activity in the CRM system and other business systems.
- Serve as point of contact for clinical/center teams and address questions or concerns from prospects and patients.
- Resolve any concerns or complaints from prospects or patients by working with them or directing them to the appropriate Oak Street employee.
- Perform other duties as assigned.
- Ability to quickly connect and influence the right people.
- Comfortable navigating external barriers to create a positive experience.
- Ability to manage multiple priorities simultaneously.
- Team player contributing to overall team effectiveness.
- Sales and/or healthcare experience is a plus.
- Fluency in Spanish, Polish, Russian, or other languages spoken by the communities we serve, when necessary.
- US work authorization.

Time TypeFull time
Pay Range$18.50 - $31.72 per hour. This range represents the base hourly rate or base annual full‑time salary. The actual base salary offer depends on experience, education, geography, and other factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
