Role DescriptionThis is a full-time, on-site role located in Fort Wayne, IN, for a Registered Nurse (RN) or Licensed Practical Nurse (LPN). The responsibilities include assessing patients health, administering medications, monitoring progress, and providing direct care in a compassionate and professional manner. The nurse will collaborate with the care team to implement treatment plans, document patient information accurately, and support patients through various stages of recovery.
Additional duties involve educating patients and families on health management while adhering to clinic policies, protocols, and standards of care.
- Active RN or LPN license in the state of Indiana with CPR or BLS certification.
- Experience in patient assessment, medication administration, and direct patient care.
- Strong communication, organizational, and teamwork skills to collaborate effectively with healthcare professionals and patients.
- Knowledge of addiction treatment, substance use disorders, and co-occurring mental health conditions is preferred.
- Ability to work in a fast-paced environment and provide empathetic care to individuals in recovery.
- Experience with maintaining detailed patient records and ensuring documentation accuracy.
- Demonstrated commitment to upholding ethical standards and maintaining confidentiality in all aspects of patient care.
