The LPN/RN reports to the Director of Nursing and is responsible for delivering, coordinating, and evaluating resident care in alignment with physician orders and individualized care plans. The role combines direct clinical duties—medication administration, assessments, documentation, direct resident support—with leadership activities such as guiding care staff, promoting safety initiatives, and ensuring effective communication with residents, families, physicians, and community leadership.
Essential Functions- Provide services in accordance with the nurse practice act and state/federal regulations.
- Work with the Director of Nursing to provide direction and oversight to care staff regarding resident needs.
- Review and revise assessments and care plans at admission, periodically per regulation, and when resident conditions change.
- Review and transcribe physician orders and deliver medical treatments as ordered.
- Initiate resident education plans and provide instruction to residents and families.
- Observe residents and communicate condition changes to physicians, proxies, and supervisors.
- Administer and document medications according to the five rights.
- Supervise and monitor medications for residents who self‑administer, if applicable.
- Document observations, incidents, interventions, communications, and clinical procedures.
- Assist residents with nutritional and hydration needs and document appropriately.
- Assist with activities of daily living (bathing, oral care, toileting, dressing).
- Assist with scheduling medical, dental, and laboratory appointments.
- Utilize proactive intervention, monitoring, and safety in rooms and rounds.
- Answer resident call lights promptly.
- Assist residents with transfers and ambulation using proper body mechanics.
- Take vital signs and assess resident condition.
- Maintain a restraint‑free environment at all times.
- Support fall prevention programs.
- Respond to life‑saving situations and follow nursing standards, policies, and protocols.
- Respond as appropriate to personal calls from other health‑care units.
- Provide first aid, call emergency personnel, and contact family members as needed.
- Review and manage assisted living and memory care resident service plans.
- Encourage resident participation in scheduled activities.
- Assist in post‑mortem care as instructed.
- Participate in medication delivery system oversight and documentation.
- Notify pharmacy and supervisors of medication issues and resolve them in a timely manner.
- Promote the physical, emotional, and psychological well‑being of each resident.
- Ensure understanding and adherence to residents’ advance directives.
- Maintain effective communication with residents’ families regarding care and wellbeing.
- Work collaboratively with staff, accept direction from management, and engage in conflict resolution as needed.
- Perform other duties as assigned.
- Graduation from an accredited nursing school and a current state license for LPN or RN.
- 1+ year of nursing experience preferred; new graduates welcome.
- CPR or BLS certification preferred.
- Proficiency with digital tools and applications, including Microsoft Office, web-based platforms, EHR systems, and HR systems.
- Position may require walking, standing, or moving throughout the community.
- May need to stoop, kneel, crouch, crawl, or climb as necessary.
- Requires effective communication and written interpretation skills.
- May need to lift up to 50 pounds independently and 200 pounds with assistance.
