Job Title and Summary

RN or LPN, PRN – Inpatient RN

Stillwater Hospice (Fort Wayne Campus) – 20‑hour part‑time or PRN schedule on nights and weekends. Hospice Home (IPU) shifts are 12 hours long. This position supports terminally ill patients in our recently renovated inpatient unit.

Base Pay Range

$24.77/hr – $41.67/hr (actual pay based on skills and experience).

Position Title

Inpatient RN – Non‑exempt, Reporting to Assistant Director of Nursing.

Position Objective

Providing nursing care for assigned terminally ill patients by assessing needs, coordinating the interdisciplinary care plan, implementing nursing interventions, making appropriate revisions, and evaluating care outcomes.

Qualifications
  • Graduate of an accredited school of nursing.
  • Bachelor’s degree in Nursing preferred.
  • One year of recent hospital or home health care experience preferred.
  • Current RN license to practice in Indiana.
  • Current CPR certification (or obtain during orientation).
  • Infusion skills preferred.
  • Ability to identify needs and emergencies of hospice patients and provide appropriate interventions.
  • Comfortable working with hospice patients, focusing on palliative care.
  • Strong interpersonal and assessment skills.
Essential Functions
  • Function independently and as part of the team.
  • Implement agency‑approved clinical policies and procedures.
  • Demonstrate understanding of hospice philosophy and consider the patient/family as the unit of care.
  • Orient patient/family to hospice services.
  • Identify patient health problems by assessing psychosocial, spiritual, physiological, and health‑behavior systems.
  • Participate in obtaining and documenting admission and assessment data.
  • Document findings accurately, in a timely manner, and on appropriate forms according to Agency procedures.
  • Deliver nursing care including teaching, direct care, case management, referral, advocacy, pain and symptom management, and initiate appropriate interventions.
  • Anticipate problems related to the patient’s impending death and pursue problem resolution, including bereavement follow‑up for family members.
  • Remain flexible for unit coverage and accept responsibility under adverse conditions.
  • Participate in appropriate staff and holiday rotation.
  • Follow orders from the patient’s physician and inform the physician and other team members of changes in the patient’s condition and needs.
  • Coordinate care and services with other health care providers.
  • Identify the need for and obtain additional orders from physicians.
  • Plan and implement patient discharge plans with patient/family, nurse, physician, and Agency goals.
  • Attend and participate in Patient Care Conferences or team meetings.
  • Recommend utilization of additional agency services and other community services.
  • Provide clinical supervision to LPNs and CNAs and instruct them on specific patient care procedures.
  • Work in a cooperative relationship with other Agency staff and seek appropriate clinical supervision.
  • Ensure proper initiation of the initial plan of care, evaluate outcomes, and coordinate revisions.
  • Participate in Agency committees and task forces and assume responsibility for continued personal growth and development.
  • Maintain patient records in a timely and organized manner.
  • Seek educational opportunities to improve clinical practice and participate in in‑service programs.
  • Adhere to all Stillwater Hospice policies and continue privacy practices as described in the Notice of Privacy Practices.
