Compensation is based on total years of experience in previous clinical roles.
In this role, you'll care for patients in the Adams, Blackford, DeKalb, Grant, Huntington, Jay, Kosciusko, Noble, Wabash, Wells, Whitley, and Allen County Areas.
You will workvarious hours as needed, including weekends.

POSITION TITLE:
HOME CARECERTIFIED NURSE AIDE
FLSA STATUS: NON-EXEMPT
REPORTS TO:
Clinical Team Leader
POSITION OBJECTIVE:
To provide safe personal care and support for the terminally ill in the hospice facility under the direction of the Patient Care Director and/or RN Case Manager.
QUALIFICATIONS:
High school diploma or GED required.
Completion of 105 hours CNA Certification program (need copy of Certificate).
Current/active CNA License
AND
Minimum six (6) months (one [1] year preferred) CNA/HHA experience, within the last twenty-four (24) months, in the home health, long-term care, or hospital setting.
OR
RN nursing students who have successfully completed courses with a grade of “C” or better, who currently possess an active CNA License, will be said to have met the above qualifications.
Must hold current CPR certification or obtain within the orientation period.
Must be able to safely perform the essential functions of the job without posing a direct threat to their health and/or the safety of him/her or others in the workplace.
Must have a current motor vehicle operator’s license and have access to an automobile with current automobile insurance coverage.
Must be able to drive independently to all patients/families/ clients’ homes.
ESSENTIAL FUNCTIONS:
Gives client care per agency policies and procedures, following the written Plan of Care.
Appropriate and safe techniques in personal hygiene and grooming that include: bed bath, sponge bath, tub bath or shower bath. Shampoo: sink, tub, or bed. Nail and skin care. Oral hygiene. Toileting and elimination needs, emptying commode and catheter bag; empty, cleanse, and reinforce ostomy bag. Normal range of motion and positioning, ambulating with the use of a gait belt. Changing linens, adequate nutrition, fluid intake and output.
Assists patients with various rehabilitation activities under the direction of a licensed therapist or RN.
Takes and records temperature, pulse, respirations, and blood pressure when assigned.
Meal preparations.
Assists patients with feeding and feeds patients unable to feed themselves.
Observes patient’s condition and reports significant changes in status to the appropriate nurse.
Computer documents care per agency policies and procedures.
Manages care of assigned patients in a timely fashion.
Maintains a clean, safe, and healthy environment in the patient care area.
