The Social Worker assesses patient/family needs and coping skills that support the patient’s unique cultural and spiritual beliefs. Provides holistic patient/family centered care, education, interventions, and community resources tailored to the specific needs. Provides crisis intervention, complex or long-term placement linkage to appropriate community resources, and participates in discharge and treatment planning. Coordinates with the care team and other coworkers to facilitate the patient's goals and achieve quality and cost‑effective outcomes.

Qualifications:

• Master’s Degree in Social Work required.
• Current Indiana LSW or LCSW license.
• Minimum 2 years professional experience with patients/families facing social complexities and cultural barriers to healthcare.
• Preferred: additional discharge planning or medical social work experience, and prior hospital setting work experience.
• Demonstrated knowledge of social work process, community agencies, services, and resources.
• Strong analytical and organizational skills to assess needs, plan interventions, and manage caseload for efficiency.
• Excellent verbal and written communication skills.
