Kingston Care Center of Fort Wayne is seeking a dedicated Speech Language Pathologist (SLP) to join our skilled nursing and rehabilitation team. In this role, you’ll help residents strengthen communication, improve swallowing, and enhance cognitive abilities while working in a supportive, resident-centered environment in Fort Wayne, Indiana.

• Evaluate, diagnose, and treat speech, language, voice, swallowing, and cognitive‑communication disorders
• Develop and implement personalized treatment plans tailored to resident needs
• Provide one‑on‑one and group therapy sessions to support functional communication and safe swallowing
• Document assessments, daily notes, and progress accurately in the EHR system
• Collaborate with physicians, nurses, occupational therapists, physical therapists, and families to ensure coordinated care
• Educate residents, caregivers, and staff on communication strategies and swallowing techniques
• Ensure therapy practices comply with Indiana regulations and Kingston’s standards of care

• Master’s degree in Speech‑Language Pathology from an accredited program
• Certificate of Clinical Competence (CCC‑SLP) preferred; CFY candidates considered
• Experience in skilled nursing, rehab, or long‑term care preferred (new grads welcome)
• Strong clinical, documentation, and communication skills
