Overview
BASIC FUNCTION: Serve as a liaison between teachers, parents, students, support staff, administrators and the community regarding educational programs, services and the issues and problems of identified students with various in areas such as attendance, intervention, behavioral modification, emotional growth, academics and health; provide related outreach, support, and guidance services.
Qualifications
QUALIFICATIONS:
Any combination equivalent to: bachelor’s degree in a social work or related field and two years of experience working with students or families in an educational, social service or related environment. Must have strong analytical, computer, and oral and written communication skills. Incumbents must possess advanced knowledge of specialized aspects, methods, processes, principles, practices, procedures and terminology used in assigned field;
must have a valid driver’s license and have the ability to transport self and other to various locations.
Essential Functions
Serve as a liaison between the homes of students identified with various needs in the FWCS school district; work with teachers, administrators, parents and others in the development and implementation of educational plans, goals and objectives for individual and small groups of students; lead and oversee related sessions and discussions.Provide outreach, liaison and guidance services to assist in resolving student issues and problems in areas such as attendance, academics, behavior and health; confer and collaborate with students, parents, teachers and others in developing and implementing strategies, goals and objectives for resolving student issues and problems.Lead and/or participate in Student Support Teams (SST);
Positive Behavior Intervention Support (PBIS), Status Offender Court Alternative Program (SOCAP) and Quality Improvement Teams(QIT); collaborate with faculty, parents, and others in the planning, development and implementation of intervention strategies to meet student needs and resolve student issues.Develop implement and provide services to facilitate and enhance emotional development, academic growth and behavioral modification for individual or small groups of students; provide crisis intervention to students, families and staff with issues and problems requiring immediate action.Interview and assess students and families in the identification of academic, social and emotional needs, issues and problems; prepare Functional Behavior Assessments (FBA); monitor, evaluate and adjust services, plans, goals and objectives in response to student needs, issues and progress.Collaborate with FWCS school district staff to identify, intervene in, and resolve student and family problems in the enhancement of student success in school; conduct individual and group discussions regarding student needs, goals and progress; plan and implement group discussion topics.Establish and maintain partnerships with community agencies to facilitate and enhance support services available to FWCS school district students and families; refer students and parents to community agencies and resources for information and assistance according to identified student needs; coordinate related services with administrators and parents or guardians.Coordinate, arrange and oversee various activities and interventions Study Connection, Lunch Buddies, Rising Star and others in response to student and family needs; coordinate additional services such as study assistance, mentors, counseling services, medical assistance and the development of extracurricular interests as appropriate.Maintain regular contact with families of identified students; make home visits to monitor student and family needs and implement interventions, educational services and related strategies, goals and objectives; assist families in compiling, interpreting and utilizing information related to student and family needs; coordinate and conduct parent enrichment sessions.Maintain and update records regarding students, interventions, strategies and related matters.Develop and facilitate conflict mediation and violence prevention.Transport individual and groups of…
