We are seeking a Family Practice Physician for a 120-day locum tenens assignment in Indiana beginning Dec 29, 2025. This full-time role involves caring for patients of all ages with a schedule alternating 8am-5pm (Monday, Wednesday, Friday) and 10am-7pm (Tuesday, Thursday). The assignment offers the opportunity to collaborate with advanced practice providers and contribute to a multidisciplinary team focused on delivering high-quality primary care.

Responsibilities and Duties
Collaborate with up to two advanced practice providers
Manage medications and treatment plans for patients
Educate patients about home treatment and preventive care
Refer patients to specialists as needed
Ensure continuity of care between visits
Complete charting and patient documentation in the EMR
Participate in peer reviews
Work in conjunction with the Chief Medical Officer and medical team
Provide care to a culturally and economically diverse patient population

Additional Information
Must hold an active Indiana medical license
Board certification in Family Medicine required
Comfortable providing care for all age groups
Basic Life Support (BLS) and Cardiopulmonary Resuscitation (CPR) certifications required

Experience with EMR systems preferred
