As a Registered Nurse for Admission/Discharge, you deliver patient‑centered care, ensuring efficient transitions for patients entering and leaving acute‑care settings. You perform specialized assessment, maintain documentation, educate patients/families, coordinate with administrative staff, and advocate for optimal clinical outcomes.
Responsibilities- Obtain a nursing assessment history and ensure proper inpatient admission; communicate special circumstances to physicians and care team.
- Complete required documentation (admission assessments, risk assessments, medication reconciliation, discharge paperwork, post‑hospital documentation).
- Teach patients/families/caregivers about the patient’s medical condition, status, treatment plan, medications, side effects, and follow‑up measures; translate complex terminology.
- Complete extra discharge documentation, education, and discharge care for special patient populations to comply with core measures.
- Ensure patients/families/caregivers receive copies of discharge paperwork and other documentation; provide referrals for ongoing care and educational materials.
- Coordinate with House Supervisors/Nursing Administration to prioritize workload and identify greatest need areas for smooth transitions.
- Demonstrate a time‑conscious awareness and ability to prioritize; distinguish between low and high‑priority activities.
- Advanced Cardiac Life Support (ACLS) certification within 6 months of employment start date.
- Basic Cardiac Life Support (BCLS) certification within 30 days of employment start date.
- (RN) Registered Nurse certification within 30 days of employment start date.
- Associate Degree, Bachelor’s Degree, or RN Diploma.
- Acute care experience with cardiac monitoring required.
