Your Specific Responsibilities:
This is a highly responsible, supervisory public health nursing position within the Office of Medical and Dental Services (OMDS) of the Okaloosa County Health Department (OCHD). The position will assess, plan, organize, and prioritize assigned clinical systems to improve efficiency, accountability, and productivity as related to the HIV/AIDS Ryan White program. Duties include completing projects for compliance with program goals to improve health outcomes.
Frequent communication with supervisor is expected and will result in a detailed performance evaluation.
• Supervise staff, including appraisals, planning and directing work, approving leave, and having authority for hiring and taking disciplinary action or effectively recommending actions.
• Manage daily clinical operations, medical and non-medical case management for the HIV/AIDS Ryan White program per FDOH and HRSA guidelines. Collaborate with supervisor and medical staff to establish HRSA performance standards and primary care standards of care in clinical practice. Develop systems to track completion of quality care standards. Utilize the clinical quality management (CQM) program to assess alignment with HHS benchmarks and quality indicators, including strategies to improve care delivery.
Develop policy or protocols with the Executive Community Health Nursing Director to ensure measures are met. Direct licensed and non-licensed staff in daily program operations, coordinate specialty medical referrals, track progress, and ensure follow-up or resolution of referrals. Gather and analyze data for continuous quality improvement. Involve staff in ongoing quality improvement efforts related to program operations and efficiency. Ensure clinic practice methods meet Ryan White program requirements, are evidence-based, and incorporate best practices.
Conduct program evaluations and write reports with findings and recommendations; implement, evaluate, and monitor corrective actions as needed. Ensure accountability of program drugs, vaccines, and compliance with regulations, registrations, and inspections.
• Research and gather data on HIV/AIDS Ryan White quality improvement strategies. Assist supervisor in creating a comprehensive quality improvement or strategic plan per HRSA & HIVQUAL guidelines. Plan should include annual goals, objectives, performance measures, staff and community engagement, and other HRSA-listed objectives. Develop systems to track plan goals. Collaborate with supervisor to implement plan objectives, action steps, and improvement activities.
Monitor and align activities with the Okaloosa County Health Department Strategic Plan and Ryan White grant objectives; track target dates and completion status.
• Manage nursing staff in enrolling new clients, providing orientation, and conducting initial education and needs assessment. Train staff on strategies to support early linkage and retention in care. Emphasize program eligibility, requirements, HIV transmission risk reduction, and adherence issues. Manage case management details to retain clients in care and ensure needed services. Ensure case management assignments and coding are maintained and coded in HMS.
Mentor program staff on strategies to provide individual-level services for clients.
• Manage internal Test and Treat program operations. Oversees the Test and Treat on-call telephone to link newly diagnosed clients to care. Coordinate Outreach components, HIV-related special events, and coordination with test sites, providers, and community groups to ensure services for individuals living with HIV. Coordinate with Area 1 surveillance officer and DIS and out-of-state agencies as appropriate. Manage follow-up on testing, reporting, and lab results, and patient contact for retesting if needed.
Assist with annual registration of county HIV test sites as needed.
