Home Health Licensed Practical Nurse

We are seeking a caring and reliable Part-Time Licensed Practical Nurse (LPN) to join our home health care team. As an LPN, you will provide compassionate, patient-centered care to individuals in their homes under the supervision of a Registered Nurse (RN). You will play a critical role in helping patients manage their health, recover from illness or surgery, and improve their quality of life while staying in a familiar and comfortable environment.



Key Responsibilities :

• Deliver direct patient care, including monitoring vital signs, wound care, and administering medications.
• Follow established care plans under the supervision of an RN or physician.
• Assist patients with daily living activities, including mobility, hygiene, and nutrition.
• Educate patients and their families about home care practices, medication administration, and health management.
• Collaborate with healthcare providers and the home health team to ensure the best possible patient outcomes.
• Document patient progress, health status, and care provided in the electronic medical record (EMR) system.
• Monitor for any changes in a patient’s condition and promptly report to the RN or physician.
• Adhere to agency policies, procedures, and state/federal regulations to ensure compliance and quality care.

Qualifications:

• Active and unrestricted Licensed Practical Nurse (LPN) license in Florida.
• Minimum of 1 year of clinical experience, preferably in home health or a similar setting.
• Strong knowledge of home health care practices and state regulations.
• Proficiency in electronic medical record (EMR) systems.
• Excellent communication and interpersonal skills.
• Ability to work independently, manage multiple patients, and prioritize tasks effectively.
• Compassionate, patient-focused, and detail-oriented.
• Valid driver’s license and reliable transportation.
