Department: GYN Oncology
Work Type: Full Time (80 hours bi-weekly)
Shift: Days, 8 AM to 5 PM, Monday – Friday, No Weekends
Minimum to Midpoint Pay Rate: $31.78 – $42.90 / hour
The Ambulatory RN Care Coordinator utilizes a collaborative process of assessment, planning, implementation and evaluation to engage, educate, promote and maintain optimal health status of assigned patients. This position functions in a collaborative partnership with the patient, physician and the care team in the office setting. This position serves in an advocacy role to ensure the delivery of quality, efficient, and cost-effective patient-centric health care services.
The Ambulatory RN Care Coordinator’s caseload consists of moderate to high risk patients.
- Provide patient-centered nursing care coordination for assigned patients in an ambulatory setting.
- Collaborate with patients, families, physicians, and care teams to plan, implement and evaluate care plans.
- Educate patients on disease management, treatment options, and health maintenance to promote optimal health outcomes.
- Advocate for patients to ensure delivery of quality, efficient, and cost-effective care.
- Manage a caseload of moderate to high-risk patients with emphasis on education and chronic disease management.
- Education: Associate's Degree in Nursing required (or higher; BSN preferred).
- Experience: Three (3) years of clinical nursing required; RN OB/GYN or Women’s Health experience preferred. Care coordination experience is preferred.

Certifications:
Registered Nurse license required. - Additional: Minimum of 3 years in a clinical setting with emphasis on patient education and chronic disease management; home health, skilled nursing, and medical-surgical nursing experience considered.
