POSITION SUMMARY:
  • Responsible for the efficient and accurate coordination of home health visits for patients
  • Maintain professional, open communication with clinicians, patients, and caregivers regarding visit schedules
  • Coordinate closely with intake, clinical management, & billing teams to ensure smooth transitions & accurate visit tracking
  • Maintain compliance with Medicare and agency-specific visit documentation and frequency rules
  • Monitor & report scheduling metrics & participate in quality initiatives to enhance efficiency and customer satisfaction.
  • Th HH Scheduler position is an in-office position
QUALIFICATIONS:
  • Detail-oriented, with strong organizational and time management skills
  • Ability to work independently and as part of a team in a fast-paced environment
  • Strong knowledge of Microsoft 365, including Outlook, Word and Excel.
  • Ability to communicate effectively in writing and verbally
  • Possess solid problem-solving skills and critical thinking skills
  • High school diploma or equivalent required
  • Experience in healthcare scheduling; 1-2 years Home Health scheduling experience preferred
  • Working knowledge of Wellsky Kinnser preferred
