Overview

Page Health & Rehab Center - Fort Myers, FL is seeking a compassionate and skilled Per Diem Occupational Therapist (OT) to join our in-house subacute rehabilitation team. The Occupational Therapist will evaluate, plan, and implement therapeutic interventions that help residents regain independence in daily living activities, enhance functional performance, and improve overall quality of life.

Responsibilities
  • Evaluate residents and develop individualized occupational therapy treatment plans in accordance with physician orders and facility protocols.
  • Provide skilled occupational therapy services focused on ADLs, IADLs, upper extremity function, cognitive skills, coordination, and adaptive techniques.
  • Educate residents and caregivers on therapeutic activities, safety strategies, energy conservation, and use of adaptive equipment.
  • Accurately document evaluations, treatments, progress notes, and discharge summaries in compliance with regulatory and facility standards.
  • Collaborate with the interdisciplinary team to support resident-centered care plans and achieve optimal outcomes.
  • Maintain a safe, clean, and supportive treatment environment.
  • Uphold all professional, ethical, and regulatory standards of occupational therapy practice.
Qualifications
  • Current Florida Occupational Therapist (OT) license in good standing.
  • Graduate of an accredited Occupational Therapy program.
  • Experience in subacute rehabilitation, skilled nursing, or long-term care preferred but not required.
  • Ability to work independently while collaborating effectively with the rehabilitation team.
  • Strong communication, documentation, and time-management skills.
  • Compassionate, patient-centered approach to care.
  • Per diem candidates must be available to work consistent scheduled shifts as needed.
