, a part of Empath Health is seeking highly motivated and compassionate Home Health PT Physical Therapists full-time for our Lee County/Fort Myers Team
. As part of our Physical Therapy Team, you will be part of a dynamic Clinical Team who ensures that our patients return and maintain their optimum level of physical functioning. At Empath Health we are:

With more than 60 years of expertise, Empath’s Home Health service line provides skilled nursing, therapy, and personal care services that help people recover and maintain independence t of Empath Health’s not-for-profit network, the program serves patients across 16 Florida counties and is accredited by the Joint Commission and the Accreditation Commission for Health Care.

• Provide physical therapy services to patient as prescribed by a physician in compliance with agency policies and procedures and federal and state regulations.
• Perform initial and ongoing assessments, including OASIS assessments at appropriate time points, to determine level of functioning and patient’s ability to perform activities of daily living.
• Develop and revise plans of care, in consultation with the physician and other care team members, which will enable patient to achieve realistic goals of mobility.
• Report to the physician and designated agency representative patient’s reaction to treatment and any changes in the patient’s condition.

• Licensure as a physical therapist in the state of Florida
• Minimum of one (1) year experience in home health care preferred
