The Primary Care Physician (PCP) role focuses on outcomes and value-based care, with strong clinical practice and cost-effectiveness for a panel of up to 450 patients. PCPs act as health coaches, help patients set and pursue health goals, and follow up regularly to drive improved health outcomes. They simplify and prioritize care to enable actionable behavior change and lead care teams toward performance goals.

PCPs operate in a culture that emphasizes love, accountability, and passion, and participate in risk and quality management programs and clinical meetings as required to promote patient health and company goals.

The PCP will function independently and in collaboration with other health care professionals, working with managers and medical directors to ensure guideline adherence and participation in relevant meetings. They will adhere to departmental goals, standards of performance, regulatory compliance, quality patient care, and policies and procedures.

ESSENTIAL JOB DUTIES / RESPONSIBILITIES
  • The PCP independently provides care for patients with acute and chronic illnesses encountered in the older adult patient.
  • The PCP takes full accountability for patient care and outcomes and appropriately seeks consultation from specialists when needed, while remaining involved in the patient’s detailed care.
  • The PCP engages with the hospitalist whenever a patient is in the hospital, regardless of whether the hospitalist works for Chen Med.
  • The PCP is responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination and documentation for patients with acute and complex chronic health needs.
  • The PCP leads their care team, including a care promoter (medical assistant), care facilitator, and care coordinator for patients able to come to the office.
  • For patients unable to come to the office (in hospital, SNF, LTC or homebound), the PCP will engage with the transitional care team and other resources available in the market.
  • The PCP participates in center management and may cover for other providers as needed; recruiting patients and providers for the center and company is also expected as needed.
  • Supervises or collaborates with an Advanced Practice Practitioner (APP) and remains accountable for the APP’s actions while employed with the company.
  • Performs other duties as assigned and modified at the manager’s discretion.
