The Hospice Admissions Nurse is responsible for presenting hospice services at the point of admission, whether at home or in a community setting. The RN initiates an individualized plan of care by collaborating with the patient, family, physician, and interdisciplinary team, anticipating patient needs and coordinating closely with related disciplines to ensure professional service delivery.
Essential Functions- Coordinate patient/caregiver care:
- Conduct admission nursing assessment to identify needs.
- Develop a comprehensive plan of care addressing therapeutic, preventive, and palliative goals.
- Maintain liaison relationships with healthcare providers and the interdisciplinary team.
- Participate in admission interdisciplinary group conferences.
- Communicate with the case‑management nurse to clarify care plans.
- Build professional relationships with external agencies and providers.
- Deliver patient and caregiver education:
- Identify learning needs and provide instruction.
- Demonstrate nursing procedures and use appropriate teaching materials.
- Encourage questions and consult as needed.
- Provide education to facility staff when admitting patients in HART facilities.
- Collaborate with the Medical Social Worker on psychosocial needs.
- Maintain accurate, timely clinical documentation:
- Synchronize laptop records daily.
- Submit all documentation per VIA Health Partners guidelines.
- Maintain medication profiles and document teaching.
- Record time/mileage logs accurately.
- Initiate Nursing Assistant care plan:
- Develop plans within the Nursing Assistant scope of practice.
- Submit plans to the administrative assistant for team communication.
- Support interdisciplinary collaboration:
- Attend and participate in assigned meetings.
- Engage in process improvement efforts.
- Communicate complex patient/caregiver issues to the supervisor.
- Maintain the professional reputation of VIA Health Partners.
- Pursue professional development and keep certifications current.
- Perform other duties as required.
Minimum Qualifications
- Diploma or Associate’s Degree in nursing required;
Bachelor’s Degree preferred. - Current, unrestricted NC/SC Registered Nurse (RN) license.
- Current CPR; CHPN preferred. Oncology nurse certification required if chemotherapy is administered.
- Minimum two years of related experience in direct care of patients with life‑limiting illnesses, such as hospice, palliative, family medicine, internal medicine, geriatrics, oncology, home health, or medical‑surgical settings.
