Overview
The PRN Nurse Practitioner (NP) for Hospice Homecare provides compassionate, patient-centered care to individuals with life-limiting illnesses in their homes. The NP collaborates with the interdisciplinary hospice team to deliver comprehensive assessments, symptom management, and support to patients and their families, ensuring comfort and dignity at the end of life.
Responsibilities- Conduct home visits to assess, diagnose, and manage acute and chronic symptoms related to terminal illness.
- Collaborate with the hospice medical director and interdisciplinary team to develop and implement individualized care plans.
- Prescribe medications and treatments in accordance with state and federal regulations and organizational protocols.
- Provide education and emotional support to patients and families regarding disease progression, symptom management, and end-of-life care.
- Document all patient encounters and clinical findings accurately and timely in the electronic medical record (EMR).
- Participate in on-call rotation as needed.
- Ensure compliance with hospice regulations, standards of care, and organizational policies.
- Master’s or Doctorate degree in Nursing from an accredited program.
- Minimum of 2 years of NP experience, preferably in hospice, palliative care, or geriatrics.
- Current and unrestricted Nurse Practitioner license in SC; DEA license required.
- Board certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology NP;
Hospice and Palliative Certification preferred. - Strong clinical assessment, communication, and interpersonal skills; ability to work independently in a home-based setting.
- Valid driver’s license and proof of automobile insurance.
