Overview
Pay rate: $51,200 Catholic Charities, Sarah's House is seeking a Case Manager. The Case Manager assesses household needs, coordinates resources based on goals, and provides advocacy, information, and referral services. The work schedule is Mon-Fri 8AM-4PM and 1 late evening 12PM-8PM.
Sarah's House is a supportive housing program offering emergency shelter and project-based supportive housing with an array of other services for families experiencing homelessness in Anne Arundel County. Serving up to 125 people daily, half of whom are children, our goal is to affect positive change in families by providing a range of services and support.
Job Duties & Responsibilities- Work with clients to primarily focus on improving quality of life in obtaining stable and permanent housing, goal setting, case planning and personal development
- Maintain casework files on each client according to policies and procedures and organize and record information into ETO and HMIS in a timely manner
- Provide support, advocacy, information and referral services to Clients of Sarah's House
- Participate in and accurately track case management progress for 30/60/90 team meetings in HMIS
- Motivate and empower clients to establish and reach goals, including obtaining information from clients such as: client’s interests, aptitudes, and abilities for developing case plans
- Ensure client confidentiality is maintained in accordance with HMIS guidelines and requirements

Specific Duties
- Sarah's House:
Acts as a liaison with various agencies including, but not limited to, Department of Social Services (DSS) and Veteran Administration (VA). - Facilitate guests in obtaining and maintaining permanent housing.
- Assist in assuring the safety and security of guests.
- Case Manager: Bachelor s degree in Human Services, Social Work, or related field.
- Two years related experience.
- Excellent knowledge of case management and crisis intervention services.
- An equivalent combination of education and experience may be considered.
- Senior Case Manager: Master s degree in Human Services, Social Work or related field.
- Five years of related experience.
- Excellent knowledge of case management and crisis intervention services.
- Demonstrates patience and composure when working with others, fostering positive relationships.
- Exhibits exceptional relational and customer service skills, building strong connections and providing top tier service.
- Acts with discretion, tact, and professionalism in all situations, maintaining confidentiality and integrity.
- Demonstrates excellent verbal and written communication skills, ensuring clear and effective exchanges.
- Displays excellent organizational skills and attention to detail, ensuring accuracy and efficiency.
- Utilizes effective conflict resolution skills to address issues and maintain a positive environment.
- Requires proficiency in using computer systems and software to perform job function, including but not limited to, basic Windows PC, web browsing (e.g., Chrome, Internet Explorer), and Microsoft Outlook. Knowledge of other Microsoft Office applications, such as Word, Excel, PowerPoint, Teams, and One Drive is preferred.
