Position Overview and ResponsibilitiesThis highly responsible position provides comprehensive assessments of applicants for nursing facility placement and home and community-based services, evaluates the Level of Care, and recommends the most appropriate placement.
Duties and Responsibilities- Coordinate and conduct comprehensive assessments of applicants for nursing facility placement and home and community-based services, evaluate the Level of Care, and recommend the most appropriate placement; participate as a member of an interdisciplinary staffing team for Level of Care determination.
- Maintain accurate and up-to-date medical information on clients through regular documentation of activities, ensuring timely and accurate data collection, reporting, and follow-up in accordance with CARES policies, procedures, and regulations.
- Input assessment data into the Client Information & Registration Tracking System (CIRTS) and maintain updated data for cases assigned.
- Participate as a member of an interdisciplinary team in making Title XIX Level of Care decisions, including physicians and other specialized professionals.
- Ensure that the Pre‑Admission Screening and Resident Review (PASRR) process is completed before a Level of Care is issued when a request is received from a state mental hospital or related facility.
- Monitor placement decisions through an annual recertification process; communicate with the long‑term care network and providers to ensure appropriate Level of Care, program recommendation, and placement recommendation.
- Provide regular medical in‑service training to professional staff in the office on topics appropriate to the aged and disabled population.
- Maintain a high level of professional CARES knowledge, including State and Federal rules and laws; improve professional knowledge, skills, and abilities through education, seminars, workshops, and policy review.
- Participate in pilot/special projects to improve assessment and delivery of services.
- Participate in disaster recovery efforts throughout the State of Florida and report to Special Needs Shelters during hurricane‑related or other emergencies.
- Perform other related duties as assigned.
- Attendance is an essential function of this position.
- Knowledge of Level of Care rules and regulations, CARES policies and procedures, the aging process, and chronic illness effects.
- Proficiency in interviewing techniques and community resource identification for elders and adults with disabilities.
- Ability to compile, organize, and analyze client information; work with clients having physical and emotional challenges.
- Strong written and oral communication skills with correct grammar, punctuation, and spelling.
- Detail‑oriented data entry into a computerized system; navigate multiple computer screens and applications.
- Basic proficiency in Microsoft Windows, Office (Word, Excel, Outlook), One Drive, and SharePoint.
- Professional and courteous demeanor to establish and maintain cooperative relationships with the public and staff.
- Ability to follow written and oral directions; read, understand, and apply complex regulations and policies.
- Ability to communicate with others to obtain and verify eligibility information.
- Analytical skills to interpret written, numerical, and verbal data; type reports and memorandums accurately and timely.
- Ability to work in a large, open office environment; lift and carry a laptop and associated documents up to ten pounds.
- Maintain confidentiality as required.
- Maintain a clear and active license as a Registered Nurse or be eligible to practice nursing in accordance with the Florida Administrative Code as defined by the Florida Board of Nursing.
- Travel is required; operate own private motor vehicle for routine travel within the assigned office’s planning and service area; possess a valid driver license.
