Position OverviewWe are seeking dedicated and compassionate Licensed Practical Nurses (LPNs) to join our home care team.
As an LPN, you will provide one-on-one nursing care to clients in their homes, assisting with medical needs, monitoring health conditions, and ensuring the highest standard of care.
Responsibilities- ✅ Provide direct patient care in a home setting
- ✅ Administer medications and treatments as prescribed
- ✅ Monitor and document patient health status and report changes to RNs or physicians
- ✅ Assist with activities of daily living (ADLs) such as bathing, dressing, and grooming
- ✅ Educate clients and their families on health conditions and proper care techniques
- ✅ Ensure compliance with care plans and follow all healthcare protocols
- ✅ Collaborate with caregivers, families, and healthcare professionals to optimize patient outcomes
- 🔹 Active Florida LPN License
- 🔹 CPR Certification (American Heart Association or Red Cross preferred)
- 🔹 Minimum of 1 year of nursing experience (home care experience preferred)
- 🔹 Strong clinical and assessment skills
- 🔹 Compassionate, patient-focused, and reliable
- 🔹 Ability to travel to client homes within assigned locations (Port St. Lucie, Cutler Bay, Fort Lauderdale)
