Summary
Under supervision of the Nurse Manager, support patients smoothly navigate different stages of care, often focusing on transitions between hospitals, home and other care settings. Create personalized care plans, assess patient needs, and coordinate with other healthcare providers to ensure a seamless and effective transition with the goal of reducing Emergency Room readmission. These navigators would provide post-discharge follow-up, education, and care coordination via phone or digital platforms, helping to bridge the gap between ED care and long-term management.
Focus on cardiac conditions.

- 1-2 years RN Experience – Preferred
- Emergency Room RN Experience – Preferred
- Transitional Care Nursing Experience – Preferred
- Case Management Experience – Preferred
- Bachelor’s degree in Nursing from an accredited school of nursing
- Registered Nurse
