Overview

Manages the ongoing, effective operation of the functions for case management and social services. Provides resource and utilization management, and discharge planning, and incorporates linkages with the community together with the various levels of care provided by Broward Health.

• Bachelor's degree obtained through a formal four-year program. BSN preferred.

• Five years of management/clinical experience with at least three (3) years relevant case management experience.
• Knowledge of clinical data interface, Inter Qual, Case Management documentation, secure faxing, Avoidable Day tracking.

• Licensed as a Registered Nurse in the State of Florida or holds a multi-state nursing license; or able to obtain license prior to start is required.
• Certified Case Manager (CCM) preferred.
• Basic Life Support.
