• Bachelor s, Master s or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• At least 1 year of previous PT experience
• At least 1 travel contract completed - preferred
• Outpatient experience - required;
  Inpatient experience - preferred
• EPIC experience - required
• Work history/resume/employment history must have 24 months of current Physical Therapist / PT experience
• Skills Checklist must be specific to the discipline listed on the job (Physical Therapist / PT)
• Professional Reference/Reference Check - charge/supervisor/nurse manager/director/CNO/COO performance evaluation/reference from within 24 months of today s date - preferred
• Driver s License - required

• Traveler will work 4 days in outpatient clinic and 1 day a week in acute care
• Traveler may be asked to float within the same discipline across local campuses
