The Larimer County Department of Health and Environment is seeking a Communicable Disease Nurse to perform disease and outbreak investigations as well as supporting our Senior Public Health Nurse and Medical Assistant in our Tuberculosis program.

In this role, you will conduct detailed case investigations for reportable conditions, with a primary focus on enteric and vaccine-preventable diseases. You will also play a key role in responding to and containing outbreaks in high-priority settings, including schools, child cares, and long-term care facilities. This position combines investigative work with hands‑on clinical duties to aid in disease mitigation efforts, including specimen collection and vaccine administration.

A portion of the workload also involves specialized case management for Tuberculosis (TB), where you will coordinate care and monitor treatment plans for patients with active TB disease and Latent Tuberculosis Infection in accordance with public health protocols.

The standard work schedule for this position is Monday through Friday, 8:00 AM to 4:30 PM. This position may be asked to respond to events outside the typical work schedule. During the initial six-month probationary period, the individual hired may telework up to one day per week, if business needs allow. After successfully completing the probationary period, the individual may telework up to two days per week, depending on business plans and operational needs.

Responsibilities
  • Perform Disease Investigations:
    Receive and review reports of notifiable conditions—specifically focusing on—by interviewing patients to determine symptom history, exposure sources, and transmission risks.
  • Lead Outbreak Response:
    Investigate and contain clusters of illness in high-risk settings such as schools, childcare centers, and long-term care facilities, providing guidance on isolation, exclusion, and environmental cleaning.
  • Manage Tuberculosis Caseloads:
    Coordinate comprehensive care for patients with active TB disease and Latent Tuberculosis Infection duties include monitoring medication adherence, assessing side effects, conducting home visits, and ensuring successful treatment completion.
  • Provide Clinical Services:
    Perform hands‑on clinical tasks to support communicable disease investigations and outbreaks, including administering post‑exposure prophylaxis, vaccines, and collecting biological specimens for diagnostic testing.
  • Documentation & Surveillance:
    Maintain accurate, confidential records of investigations and clinical encounters, entering data into state surveillance systems to track community health trends.
