Registered Nurse

Registered Nurse - Home Health (Full-Time)

What You'll Do

• Conduct patient assessments and ongoing re-evaluations
• Develop and manage individualized plans of care
• Provide skilled in-home nursing services
• Complete required documentation, including OASIS and care summaries
• Coordinate care with physicians and interdisciplinary teams
• Educate patients and families on health management
• Participate in on-call coverage as scheduled

Qualifications

• Graduate of an accredited nursing program
• Active RN license in the state(s) of practice
• 2+ years of nursing experience preferred

Schedule

• Full-time (40 hours/week)
