Primary Responsibilities

Conduct comprehensive assessments of hospice patients.

Collaborate with primary care providers to develop and implement individualized care plans.

Continuously reassess patient needs to ensure appropriate, cost-efficient levels of care.

Arrange non-medical support services (housing, transportation) to ensure treatment compliance.

Coordinate care transitions with physicians, social workers, discharge planners, and claims professionals.

Engage specialty resources as needed to achieve optimal patient outcomes.

Maintain detailed records of clinical, functional, and fiscal outcomes.

Identify opportunities for health promotion and illness prevention.

Intervene quickly to prevent adverse patient outcomes when possible.

Serve as a patient advocate, ensuring privacy and confidentiality.

Qualifications

Associate’s or Bachelor’s Degree in Nursing or related field.

Current, unrestricted RN license in the state of Minnesota.

Proven ability to analyze complex medical information and make sound clinical decisions.

Hospice experience is required.

Ability to manage multiple priorities with empathy and professionalism.

Experience coordinating care across interdisciplinary teams.
