RN / Registered Nurse - Hospice Admissions
Generations Hospice is seeking a passionate, dedicated Hospice Admissions RN to provide expert, patient-centered care at a critical time in people’s lives. This full‑time role offers a $5,000 sign‑on bonus: 50% paid at 6 months and 50% at 1 year. The schedule is Monday‑Friday 8 AM‑4:30 PM with on‑call responsibilities.

Location:
1028 S. Main Street Suite C, Fond du Lac, WI 54935. Coverage area: travel throughout the territory is required.
- Visit patients in homes, hospitals, nursing facilities, and other settings referred for hospice care.
- Explain hospice services, discuss goals and care with patients and families, answer questions, and obtain necessary consents and legal documents.
- Obtain medical history, current medications, and symptoms.
- Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines.
- Coordinate the start of services (nursing, aides, equipment, medications).
- Collaborate with interdisciplinary team.
- Maintain accurate and timely documentation.

- Graduate of an accredited nursing school with current licensure to practice in the state of operation.
- One year of hospice nursing experience.
- Must be a licensed driver with an insured automobile in good working order.
- 3+ years of hospice or similar setting experience (long‑term care, intensive care, or emergency room).
