$/yr – $/yr (based on experience). Compensation follows a pay‑per‑visit model. In addition, 21

- Provide hands‑on patient care within RN scope, including PICC lines, wound care, NG/OG/J tubes, catheter care, drainage systems, tracheostomy care, blood draws, ostomy management, injections, IV antibiotic/TPN administration, and compression wraps.
- Deliver direct patient care as defined by the state Nurse Practice Act.
- Implement the plan of care initiated by the Case Management RN.
- Complete accurate and timely documentation, within 24–48 hours.
- Assess and provide patient and caregiver education related to diagnosis and plan of care.
- Assist the RN in performing skilled procedures and maintain equipment and materials.
- Coordinate home‑health services and report interdisciplinary needs to the Case Management RN or other disciplines.
- Use equipment and supplies effectively and efficiently.
- Participate in personal and professional growth and development.
- Execute duties assigned by the Case Management RN.
- Communicate patient information, prioritize care, and report timely to the Case Manager.
- Document patient needs, problems, capabilities and response to interventions.
- Record incidents that affect patient care.
- Report pertinent information to the Case Management RN and providers.
- Attend weekly case conferences as required.
- Maintain patient privacy in medical records and during care.
- Maintain professionalism and seek opportunities to enhance clinical knowledge.
- Meet productivity standards set by the Case Management RN.
- Complete Start‑of‑Care assessments, documentation, and plans of care with the Case Manager.
- Participate in on‑call duties as required by weekend rotation.
- One (1) year of recent nursing experience (waivable for internal candidates or new grads).
- Preferred:
One year of home‑health experience. - Plus:
Experience with high acuity patients. - Highly desired:
Experience working with an interdisciplinary team. - Graduate of an accredited nursing school (Bachelor’s degree).
- Current California RN license.
- Current Basic Life Support (BLS) card.
- Valid California driver’s license with acceptable driving record.
- All licenses and certifications must be current at time of hire and sustained throughout employment.
- Self‑directed, critical thinker, outstanding interpersonal, organized, computer literate; excellent observation, verbal and written communication, problem‑solving, basic math, and nursing competencies.
- Maintain annual compliance training and certifications.
