SubstanceAbuse and Mental Health (SAMH) Program
The Office of SAMH is statutorily responsible for the planning, evaluation, and implementation of a comprehensive statewide system of care for the prevention, treatment, and recovery of children and adults with serious mental illnesses or substance use disorders.
Northeast Florida State Hospital (NEFSH)is a 613-bed, licensed mental health treatment facility that provides person-centered services to adults suffering from severe and persistent mental illness. The hospital, located in Macclenny, was established in 1959 and is the largest state operated civil treatment facility.

This is a highly responsible and professional position serving as a Human Services Worker II within the Transportation unit. The ideal candidate will be a collaborative team player who works seamlessly across all sections within the program office. This role requires the use of independent judgment, a proactive approach to responsibilities, and exceptional attention to detail. Specific Duties and Responsibilities- The incumbent is responsible for assisting Administrative Services by supporting the mission, vision and values of Northeast Florida State Hospital and delivering core support services. This position is assigned to the Transportation Department to provide assistance in the daily operation of the facility.
- Medical Appointments:
Transports individuals off grounds to community physicians and other medical facilities to reduce controllable expenses. Assists in keeping the people we serve calm when transporting to and from appointments. Picks up the people we serve meals if their appointment is during meal time. Documents behavior of the individual while off grounds, if necessary. - Transit to/ from ACLF or mental health facility when discharged back to the community. Picks up the individual from the living area, checks for all luggage, paperwork, and medication as needed, assists staff with unloading and check-in process, obtains signatures on all paperwork including Transportation Discharge Checklist, and returns forms to the appropriate office.
- Operate all vehicles within the posted speed limits on NEFSH campus and community highways. Follow assigned schedule efficiently so appointments are timely met.
- Transport the people we serve to their work areas and Recovery Centers as scheduled.
- Maintain vehicle condition: keep vehicles clean, fueled, and perform maintenance checks to reduce costs. Notify supervisor and hospital mechanic of any problems and maintain a neat, accurate and legible Vehicle Usage Record.
- Deliver legal documents to community lawyer or courthouse, blood to community labs, pickups of x-rays or reports from community hospitals or physician’s office.

- Knowledge of methods of instructing and providing care to others.
- Ability to understand and apply methods and practices used in medical, psychiatric, nursing, child care, or working with developmentally disabled persons.
