RN- Skilled NIGHTLY CASE

Overview

The Visiting Nurse Association of Florida (VNA) is a 50 year old home health agency that is growing and hiring Registered Nurses (RN) in the Palm City area.

As an RN
, you will work with referral sources from the healthcare community to implement services to meet the needs of individuals and families in their homes. You will be responsible for completing physical assessments and determining any patient home care needs, and assist in developing and implementing that plan of care for each patient while using VNA of Florida's case management model.

• Complete physical assessments and determine patient home care needs.
• Develop and implement the plan of care for each patient using the agency's case management model.

• Trach/Vent and GT/Supra Pubic – must have experience
• Active Registered Nurse License required
• Minimum of 1 year clinical experience
• Home Health/HCHB experience a plus
• ICU experience preferred
• Available every other weekend
