Job Summary and QualificationsAs a Registered Nurse for Admission and Discharge, your top priority is delivering exceptional patient‑centered care. You are dedicated to ensuring a seamless and efficient transition for patients coming in and out of acute care hospital settings. You will provide specialized assessment skills to ensure that patients are placed in the appropriate care areas and discharged. We offer education and post‑hospitalization care as necessary, advocating for patients, families, and caregivers while embodying the organization’s vision, mission, and values.
The goal is to give an unmatched patient experience and attain optimal clinical outcomes.
- Obtain a nursing assessment history and ensure proper inpatient care unit admission. Take appropriate action when needed, such as communicating special circumstances to physicians and/or care team or reporting indications of abuse or neglect.
- Complete and facilitate the completion of required documentation, such as admission assessments, risk assessments, medication reconciliation, discharge paperwork, and post‑hospital documentation.
- Teach patients/families/caregivers about a patient’s medical condition, patient status, treatment plan, medications and possible side effects, and follow‑up measures. Translate complex medical terminology to ensure complete knowledge.
- Complete extra discharge documentation, education, and discharge care for special patient populations to ensure compliance with core measures for discharge.
- Ensure patients/families/caregivers receive copies of discharge paperwork and any other documentation needed, such as referrals for ongoing care, educational materials, etc.
- Coordinate with House Supervisors/Nursing Administration to prioritize workload and identify greatest need areas to ensure smooth patient transitions.
- Demonstrate a time‑conscious awareness and ability to prioritize. Distinguish between low and high‑priority activities.
- Advanced Cardiac Life Support (ACLS) must be obtained within 60 days of employment start date.
- Basic Cardiac Life Support (BLS) must be obtained within 30 days of employment start date.
- Registered Nurse (RN).
- Associate Degree, or Bachelor’s Degree in Nursing.
